Tóm tắt

Đặt vấn đề: Gián đoạn là một trong những nguyên nhân gây ra lỗi trong quá trình thực hiện thuốc. Các giải pháp giảm gián đoạn được nhiều nghiên cứu chứng minh góp phần giảm gián đoạn.

Đối tượng và phương pháp nghiên cứu: Nghiên cứu bán thực nghiệm tại 3 khoa lâm sàng của một bệnh viện hạng đặc biệt từ tháng 3/2020 – 10/2020 với các giải pháp can thiệp là tập huấn kiến thức, thái độ và thực hành quản lý, ứng phó gián đoạn, thiết kế áo khoác cảnh báo thời điểm thực hiện thuốc cho điều dưỡng, cung cấp tờ rơi và hướng dẫn cho người bệnh các hoạt động phối hợp góp phần giảm gián đoạn khi điều dưỡng thực hiện thuốc. Đối tượng bao gồm 52 điều dưỡng, 148 người bệnh, 264 lần quan sát điều dưỡng thực hiện thuốc trước can thiệp và 345 lần sau can thiệp, với mục tiêu đánh giá hiệu quả các giải pháp giảm gián đoạn trong việc thực hiện thuốc.

Kết quả: Điểm trung bình kiến thức, thái độ và trải nghiệm quản lý, ứng phó gián đoạn của điều dưỡng trước can thiệp là 87,1±10,9, tăng so với sau can thiệp là 103,8±10,2 (p<0,001); Có sự khác biệt về thái độ của người bệnh trước và sau can thiệp (p<0,01). Tỉ lệ gián đoạn trong thực hiện thuốc giảm 27,1% sau can thiệp (p<0,01).

Kết luận: Gói giải pháp can thiệp đa yếu tố cải thiện kiến thức, thái độ và trải nghiệm quản lý, ứng phó gián đoạn của điều dưỡng. Gói giải pháp này cần được duy trì và mở rộng để giúp giảm gián đoạn, từ đó góp phần giảm sai sót trong quá trình thực hiện thuốc
