Abstract

Metformin-associated lactic acidosis (MALA) is rare but is serious and related to a high mortality rate. The reported case is a severe lactic acidosis case in a COVID-19 patient treated with metformin. The patient responded poorly to intravenous bicarbonate but recovered well after emergency hemodialysis. SARS-COV-2 infection has been reported to significantly increase the risk of lactic acidosis in patients with type 2 diabetes treated with metformin. The associated mechanism between lactic acidosis and metformin is unclear. This condition is common in patients with impaired renal function. Hemodialysis should be considered early in cases of severe lactate acidosis suspected of being associated with metformin.
