Abstract

Objective: Description of clinical and paraclinical characteristics of patients undergoing platelet transfusion, along with an investigation into the indications, utilization, and response efficacy of platelet components at Military Hospital 175.

Subjects and Methods: This is a retrospective study on 278 patients with an indication for platelet transfusion from January 2022 to December 2022 at Military Hospital 175.

Results: The majority of patients undergoing platelet transfusion were male (72.4%), with a median age of 56 years (IQR 38.75; 65) and comorbidities (79.5%). Patients presented with symptoms of anemia (70.5%) and hemorrhage (71.6%), with a coagulation disorder prevalence of 37.4%. The percentage of patients with platelet counts below 20 G/L was 42.8%, and the hemorrhage rate in this group reached 47.2%, a statistically significant difference with p < 0.001. Platelet transfusion efficacy achieved 98.3% of the prescribed indication, with self-prepared platelets accounting for 40.1%. Most platelet transfusion indications were emergencies, planned transfusions represented only 15.5%. The departments with the highest platelet transfusion indications were Intensive Care (51.8%), followed by Oncology (16.6%) and Hematology (10%).

Conclusion: The patient population was predominantly male, middle-aged, and presented with multiple comorbidities. Clinically, most patients showed symptoms of anemia and hemorrhage. Platelet transfusions were primarily indicated for patients with thrombocytopenia below 20 G/L and active bleeding. The platelet response rate was relatively high in relation to indications (98.3%), however, the rate of self-prepared platelets remained low (40.1%). Platelet usage was highest in the Intensive Care Unit and Oncology Institute.
