Abstract

Objective: To study the result of non-operative management of blunt liver trauma at Military Hospital 175.

Subject and method: Patients with blunt liver trauma were managed non-operatively from 1/2019 to 9/2021. Descriptive retrospective and prospective study without comparative group.

Result: 58 patients with blunt liver trauma was treated conservatively. Mean age 34.3±14.8 years old; male/female ratio was 3/1; traffic accident 81.1%; stable hemodynamic on admission 100%; clinical symptoms: abdominal pain 100%, abdominal distension 44.8%, abdominal wall injury 36.2%; associated injury: thoracic trauma 29.3%, extremity 19.0%, right kidney 10.3%, right adrenal gland 6.9%, spleen 5.2%. Liver enzymes 352.3±278.9 U/l, AST 467.5±400.9 U/l; ultrasound detected parenchymal injury 81.1%, free intra-abdominal fluid 83.0%; CT detected parenchymal injury 100%, free intra-abdominal fluid 74.1%, lesions in right hepatic lobe 94.8%, grade II-III 69.0%. Length of hospital stay 9.6±4.5 days; the complication rate was 1.7%; blood transfusion rate 13.8%; the overall success rate of non-operative management was 98.3%.

Conclusion: Non-operative management of liver rupture due to blunt abdominal trauma was safe, no dead. The complication rate was low, the success rate was high (98.3%).
